Tracheotomy.—The belief that tracheotomy is an easy operation is byno means justified by the authors experience. Occasionally, on a slenderneck, and when there is competent assistance to be had, it is a simpleenough procedure. But in most cases, especially on children, it calls forthe best qualities of an experienced and cool surgeon. The necessity of tracheotomy having become manifest, three require-ments are to be fulfilled. First, infection of the wound has to be avoided ; 98 RULES OF ASEPTIC AND ANTISEPTIC secondly, unnecessary hfemorrhage has to be guarded against; and, tliirdly,the trachea has to be properly incised, and the cannula properly introducedand secured. The risks of the operation are not inconsiderable, hence intubation ofthe larynx, a much simpler, easier, and more physiological procedure, must be declared to be far preferable to tracheotomywhere its application is proper, as in croupouslaryngitis. For the removal of foreign bodies and in casesof tumor of the larynx, tracheotomy will remainthe proper measure. Avoidance of infection ofthe wound from within orwithout is an ever importantmatter in all laryngeal op-erations. But it is especial-ly important, and also moredifficult, in cases where theoperation is done in the pres-ence of an infectious process,as, for instance, diphtheriticcroup, where the extensionof the septic condition tothe external wound s
