The following intrathoracic conditionsmav simulate lung tumors more or lessclosely and must be considered in the dif-ferential diagnosis. Mediastinal tumor can usually be dis-tinguished by the roentgenologic appear-ance. Oblique examination will show a dis-tinct mass in the mediastinum, and exam-ination in the antero-posterior direction the shadow directly continuous with Diagnosis of Primary Tumors of the Lung lOI that of the mediastinal structures. IntratJioracic goiter and cystic tumors ofthe mediastinum give a rather characteristicroentgen picture. (Fig. 7.) Giinuna of the lung is a condition havingmuch the same roentgen appearance as ma-lignant tumors (Figs. 8 and 9). The diag-nosis depends upon the Wassermann testand the disappearance of the mass underanti-syphilitic treatment. Caseous pneumonia may readily be mis-taken for malignancy. The early symptomsmay be c[uite indefinite just as they are inmalignant disease, and the roentgen picturemay not be distinctive. The irregular tem-perature, gradually becoming higher as thedisease progresses, the presence of rales, thefinding of tubercle bacilli in the sputum, andthe character of the sputum may serve todifterentiate from lung tumors. There are certain inflammatory processesoccurring at or near the hila of the l
