In the male, whether OPERATIONS ON THE KIDNEY AND URETER, 121 this point be in the abdominal or the pelvic course of theureter, the calculus can be reached and removed retroperi-toneally, by prolonging the anterior or lower end of theincision in the direction mentioned before. In the female the same procedure answers for the ab-dominal and first portion of the pelvic course of the ureter;but for the removal of a calculus from the ureter within thebroad ligament, especially near the uterus or vagina, thevaginal or sacral route is the best. If situated quite atthe vesical orifice of the ureter, it can be extracted throughthe bladder after dilating the urethra and, if needful, slightlyincising the ureteral orifice.
