Vaginal Myovwtoiny.—If the tumor is too large to be removedthrough the vagina by enucleation, it often becomes necessary toremove the growth by fragmentation or morcellenient. Pean, whopractised this operation on a large scale and carried its indicationsto their utmost limits, successfully removed through the vaginaby this method many large myomata which other surgeons wouldhave attacked by an abdominal section. The operation is especiallyintended for sessile and interstitial myofibromata of the body of theuterus. Pean employs in this operation forceps of special construction. Fig. 353.—Peans forceps, serrated and with teeth, for morcellation of myofibromata. (Fig. 353), with which he performs morcellation of the tumor. It isthe object of the operation to remove the tumor piecemeal, and notby enucleation. The tumor is attacked from the centre, and fragmentsare removed in the direction of the periphery until all tumor-tissuehas been removed. The first step of the operation consists in rendering the tumoraccessible. This is done by detaching the cervix in the same manneras in performing a vaginal hysterectomy, only tliat opening of theperitoneal cavity is carefully avoided. Hemorrhage during this stepof the operation is controlled by the use of hemostatic forceps. Afterthe cervix has been isolated it is incised, and the incision is carried intothe uterus as far as the tumor. The tumor is then carefully locatedwith the finger, after which the morcellation is begun in the centreof the growth. The vagina is retr
