It is now recognised that o the tumours of the gland which areassociated with acromegaly are in thefirst instance of a glandular (adeno-matous) type ; although they may laterbecome of a malignant or of a cysticnature and lead to the destruction of theglandular tissue. It is therefore heldby most authorities that the char-acteristic symptoms of acromegaly areoriginally due to hyperpituitarism, increased function, although it maybe that there is also some degree ofperversion of function. In later stages effects of destruction may un-doubtedly become apparent and supersede the symptoms of the tumour of the gland is from the first malio-nant, but. FIG. 77.—Case of pituitary gigantism.(Gushing.) The patient was 36 years ofage and 8 feet 3 inches high. Noticethe large elongated face, the dull heavyexpression, the long limbs, narrow chest, enlarged joints, enormous hands and feet, and the deficiency of hair (hypotrichosis). The last is not a constant feature. 1 Marie appears more recently to have modified his views ; inclining on the whole tothe belief that it is a perversion and not a suppression of the secretion which leads to theabnormal growth. 110 The Endocrine Organs even then many of the cells tend to resemble those of the normal gland(malignant adenoma) and symptoms of acromegaly may still be one has yet succeeded in producing the effects of hyperpituitarismexperimentally, either by operation or by feeding with the gland. It must be stated that by no means every case of tumour of the pituitaryis accompanied by the symptoms of acromegaly
