A manual of diseases of the throat and nose : including the pharynx, larynx, trachea, oesophagus, nose and naso-pharynx . tly, he vomited violently, and after bringing uplarge quantities of blood, fell insensible on the floor. On examining himwith the laryngoscope, a piece of bone was seen to be lodged horizontallyin the larynx, just above the level of the vocal cords, in such a mannerthat it completely blocked up the anterior third of the laryngeal canal(Fig. A). The bone could not be moved with a laryngeal probe, and afterrepeated unsuccessful efforts to seize it with forceps, the symptoms being-very urgent, tracheotomy was performed. On October 5th Sir WilliamFergusson attempted, with various instruments, to remove the bone ; butit was so impacted that it could not be dislodged. That eminent surgeon Lancet, Dec. 2, 1870. 2 See page 237. NERVO-MUSClLAR AFFECTIONS OF THE LARYNX. 303 recommended that, if after a delay of a few days, the bone was still im-pacted, the thyroid cartilage should be divided, and the bone removedthrough the wound. On October 14th I succeeded in passing a blunt hook tin n lamina. v behind and below the bone, and in this manner brought upof bone about half an inchacross in each direction. Onthe following day a smallpiece of bone was seenprojecting from the rightventricle (Fig. B). Afterpassing an instrument be-low, and slightly movingthe fragment, the patientcoughed up a piece of bone, about half an inch long, anda quarter of an inch wide, -r ?--? .: covered on one side with u X gristle. On putting the pie-ces of bone together, theymade altogether a laminathree-quarters of an inchlong and half an inch wide(Fig. C). This had beenimpacted horizontally withits long diameter across thelarynx in such a mannerthat about three-eighths of an inch had extended into the right ventricle, and rather less than a quar-ter of an inch of the bone into the left ventricle ; in this way the bonecould not move to the smallest extent either upward or downward.
