Theboiler was one of a battery oftwo, both of which could be cutoff from the water heating systemby valves on the inlet and outletpipes. It is thought that thesetwo valves on the wrecked boilerhad not been opened before plac-ing the heater in service and thatthe safety valve had become in-operative, thus permitting an ex-cessive pressure to build up andcause an I92I.
