You may getto the floor of the antrum by taking out the decayed teeth, and ifthe sockets of the teeth are absorbed, which is generally the casewhen they have long been in a bad state, you may make a suffi-ciently dependent opening for the escape of the matter. The teethhaving been extracted, the opening may be made by means of aperforator, a triangular-shaped instrument of sufficient size. Theopening must be large, so that it may not be readily obstructed byany swelling of the gums, or anything of that kind. If the crown of atooth is present, and is not over decayed, it may be removed withforceps. * You take out whichever tooth is decayed, perhaps onelarge or two small grinders, and all the stumps that are present,with a view of relieving the irritation, getting more readily at theantrum, and making a sufficiently large opening. For the latterpurpose you use such an instrument as this, and the tooth beingremoved there will be no difficulty in carrying this up into thecavity, as you perceive I have done in the subject before us. Thatis the proceeding you would adopt where the teeth are decayed,but you would never take out a sound tooth with the view of get-ting an opening into the cavity of the antrum.
