I now find that the initial mistake was insupposing great pressure to be necessary and inendeavouring to devise au apparatus sufficiently stronoto withstand it. A few experiments with • Harrisonspneumatic armlet soon convinced me that no greater. pressure than could bo procured by the simple hand bulbof that instrument was necessary, and indeed, with onlyBlight modification, this apparatus makes an excellenttourniquet. To make it efficient for surgical purposesI found it necessary to introduce a clip (A) between thevalve and the ])a<l, otherwise enough air escapes to renderthe tournieet ineffective within a short time. It is alsonecessary to use a strap and buclde with loops, to jireventtlie pad getting out of position when fully inflated, andI have found it an advantage to have the inner surfaceot the pad covered with soft wash-leather. To get the best results with any typo of toumiqnet, the limb shouldbe first emptied of blood, and this is best attained by theapplication of a rubber bandage after Esmarchs tourniquet is thin applied round the limb at therequired site, the leather-covered surface being iu directcontact with the skin.
