Bunion surgery. Image 12 of 12. Pinned foot after undergoing chevron osteotomy and distal phalanx bone (end toe bone) removal surgery in order to correct a bunion. Bunions (hallux valgus) are swellings of the joint between the big toe and the first metatarsal bone, which causes the base of each big toe to project outwards and their tips to point inwards. A chevron osteotomy is where the first metatarsal bone of the big toe is cut, using a V-shaped incision, repositioned and screwed into place.
