NEW GROWTHS OF THE VULVA 357 Epithelioma (Sauamous-celled Carcinoma). The vulva is therarest site in the female genitalia for cancer to develop. Vulvalcancer is a disease of advanced age ; its starting-points are the clitoris,labia majora, labia minora, and the perineum. The growth is char-acterized by extensive new formation of tissue and by a tendencyto early superficial ulceration ; there is a diffuse infiltration of thesurrounding tissues, with involvement of the inguinal lymphatic EiG. 170. Epithelioma of the Clitoris. There is no uniformity of opinion asto which site, the clitoris or the labia, is the commoner as the starting-point of cancer. Individual experience differs on this point. In ourown case we have seen more examples of cancer of the labia thanof the clitoris. When the clitoris is primarily affected, the disease isseen to start on the prepuce as a hard nodule which quicklj^ ulcerates ;there is diffuse infiltration of the smrounding tissues and the inguinalglands are early involved, but not in all cases. The course of thedisease may be rapid or chronic, but as the clitoris is very sensitivethere is a better chance of early detection than when cancer starts in thelabia.
