The perineal band is of strong leather, coveredwith buckskin, and well stuffed with wool: nearthe place where its two ends are buckled together21 242 MINOR SURGERY, on the limb, a little leather pocket is sewed forreceiving the upper end of the external splint. Application. — The patient being properly dis-posed upon the bed, a piece of linen, of the lengthof the limb and about three-quarters of a yardbroad, \edporle-attelle,OY splint-cloth, is passedunder the limb, lying upon five tapes. In thenext place, the perineal band is applied, the sur-geon having previously surrounded the upperpart of the limb obliquely with a cushion of wad-ding, four fingers breadth wide, and the lengthof the thigh-belt, or with the junct-bag, in orderto moderate the pressure of the latter, and renderit more supportable. The hollows of the sole ofthe foot and lower part of the leg are filled upwith wadding or tow, and the foot-piece, is fast- Fig. ened to the former by means of the soft leatherstraps attached to its under surface, which passround the lower part of the leg: should, however, these straps appear insufficient to fix the iron soleto the foot firmly, an extra band may be appliedin the same manner, of calico or linen. That done, the surgeon proceeds to the reduc-tion of the fracture, and afterwards adapting theupper extremity of the splint to the pouch of the MINOR SURGERY. 243 perineal band, the foot-snpport being connectedwith the splint, the cushions, and the anterior andinternal splints are to be applied, and the wholefixed by means of the tapes, as in the ordinary-apparatus for fractures of the thigh. Lastly, byturning the winch, the iron sole is lowered, draw-ing the foot, to which it is attached, along withit; and the superior extremity of the splint ispushed upward, and the member elongated to thenecessary extent.
