Occasionally the increase in size of the true. Fig. 70.—Hydronephrosis. pelvis may be the only apparent change, and the outlineof the calyces may remain practically normal. In Fig. 70the enlargement of the right true pelvis is the predominatingfeature. The papillae usually projecting between the calycesare almost effaced, in contrast to those in the normal leftpelvis. The major calyces are greatly abbreviated, withthe exception of the lowest, which is evidently incom-pletely distended. In Fig. 71 the dilatation in the left 104 PYELOGRAPHY true pelvis is the predominating feature. Although themajor calyces are probably not fully distended, they arefairly well outlined and are but slightly dilated, while theterminal irregularities are effaced. The intercalyx papillaeare unusually well preserved. Considerable difficulty may be found in differentiatingthe early hydronephrosis from the large normal pelvis, since the outline of either the true pelvis or of the majorcalyces in a normal kidney is not infrequently of unusual
