A text-book of the diseases of the ear and adjacent organs . ch extend over the posteriorhalf of the membrane are frequently bulged forward by the air-douchefor a short time to such an extent that the handle of the malleus ismostly hidden by this bubble-shaped protuberance. The free cicatrices alone seldom produce much disturbance inhearing, and we are only justified in assuming that the deafness is 378 CHRONIC PURULENT INFLAMMATION OF THE MIDDLE EAR. due to the cicatrix, if after it is forced outward by the air-douche(when catarrhal disturbances are wanting) an improvement inhearing occurs, which disappears when the cicatrix again retracts. The cicatrices lying upon the promontory are usually accompaniedby deafness. Often a marked improvement in hearing occurs if thecicatrix is forced outward by the air-douche. It is only whenthere is a discontinuity between the incus and stapes, in which thecicatrix lying upon the stapes transmits the waves of sound directly, that the bulging of the cicatrix outwards can produce a transientdecrease in Fig. 204. —Diagrammatic Section inwhich the position of an exten-SIVE Cicatrix of the MembranaTympani, in contact with theInner Wall of the TympanicCavity, is illustrated. a, Ledge-shaped remnant of the mem-brane ; b, c, The lateral portions ofthe cicatrix, extending from the rem-nant of the membrane to the innerwall of the tympanic cavity ; d, Por-tion of the cicatrix applied t<t theinner wall of the tympanic cavity.
