WASHINGTON (Sept. 17, 2020) ̶ In this photograph is an MD Helicopter Inc 369HM that crashed on January 15, 2018 in Perrysburg, OH after departing in snow, icing, and during instrument meteorological conditions. As a result of the crash, there were two fatalities. The NTSB determined the cause of the accident was a loss of engine power due to snow or ice ingestion at an altitude that was insufficient to allow for engine re-ignition.
