. Archives of physical medicine and rehabilitation . though there resultsrelaxation in the haustral changes characteristic of the spas-tic or dyskinetic form of constipationmay be associated with this spastic con-traction of the cecocolic sphinctericarea. It is necessary to differentiatespasm of this sphincteric area from adistortion in the colonic form, whichresults from the presence of adhesionsand from benign stenosis, in this Disease It is pretty generally understood thatareas of narrowing in the lumen of thegastro-intestinal tract are sites of pre-dilection for organic disease. The por-tion of the colon corresponding to thesite of the cecocolic sphincteric areaappears to be a favorite site of ulcera-tion, with fibrous stricture and perico-litis and of tuberculous and malignantdisease. This portion of the colon is onlysecond to the pelvic colon in the orderof frequency of involvement by dis-ease. The symptom complex of cecalstasis may be extrinsic and congenitalin origin, as when it is due to Jack- K h. 416 THE CECOCOLIC SPHIXCTERIC TRACT—HIHSCH sons membrane or to an abnormal at-tachment of the omentum to the frontof the cecum and ascendmg colon, orintrinsic in origin when due to an in-flammatory thickening and sclerosis ofthe cecocolic sphincteric area. Perico-litis in this cecocolic region may resultfrom a primary colitis or from a sec-ondary infection due to cecal stasis, produced in both instances by extrinsiccauses. It would appear from the publishedrecords of malignant disease of thecolon that carcinoma of the ascendingcolon is relatively rare, though thececum is considered to be a very fre-quent site for malignancy. Analysis of tumors of the colon, situated at a point distal to the ileo-cecal valve, which have been classedas cecal, will show when anatomicalboundaries above described are recog-nized, that the majority of these tumorsare really located in the sphinctericarea.
