After firmly fixing the frag-ments together close the incision, apply a steriledressing, and over all plaster-of-Paris bandages. Thisdressing should remain on for about three weeks ifall goes well, then be cut, the fracture carefully ex-amined, and if in good position and union fairly be-gun, the plate or nails may be removed and anotherplaster bandage applied. This last bandage shouldremain on for two weeks longer, and then the ex-tremity be managed as usual. Dr. John B. Eobertshas recommended sub or transcutaneous nailing inoblique fractures. The nail punctures make thedanger of infection about as great as it is when anincision is used, and I think it is a great advantageto be able to remove the shreds of muscle, blood clots,. Fig. 44. Nickel plated steel plate with ivory pegs to hold it in place- etc., from the ends of the fragments, and be sure byactual sight that the fragments are accurately fittedtogether before applying the nails or plate. I amaware that these recommendations will strike manyphysicians as radical, but I firmly believe they are Practuees of the Bones of the Leg. lfil proper and right, and the very besl treatment whenone cnn have proper surroundings and appliances. No one should attempt these operations who is notfamiliar with thorough aseptic technique, however. Next to the open method with local fixation, I be-lieve the best treatment for these very oblique lateraldisplaced fractures, below the middle of the tibia, isan old-fashioned fracture box with movable this fracture box, by elastic extension from themalleoli and heel by means of adhesive strips andrubber tubing, and by varying the padding in thebox, especially by the facility of examining
