Pathology. — Thequite considerablethickening of the uter-ine mucosa is noticedfirst of all. It mayreach a thickness of 1cm and, indeed, theformation of mucous polypi, single or multiple, may occur during thecourse of the chronic irritation, and these may project into the lumenand may even coalesce with each other (see Fig. 96). Microscopically one finds as the most predominant changes those ofthe glandular apparatus. Mostly an increase in number, hyperplasia, aswell as an enlargement, hypertrophy of the individual glands takes hyperplasia causes in the microscopic picture quite peculiar figuresas a result of the marked surface proliferation of the glandular tubular glands may divide in their depth into several finger-shapedprocesses, so that a pseudo-acinous arrangement arises. Or the gland-ducts yield laterally with the increasing hypertrophy whereby manifold. Fig. 97.—Endometritis Glandularis Hypertrophica. Ser-rated form of glands. (Authors preparation. Zeiss,Obj. AA, Oc. 2.) PATHOLOGY OF THE VAGINA AND UTERUS 171 spiral forms (corkscrew or serrated forms) of the glands arise (seeFig. 97). Very often an invagination of the ducts occurs on account of theirvigorous growth in length, so that in a section several circular epithelialcrowns packed into each other appear. The hypertrophy and hyperplasiaof the glands of the mucosa may become so extreme that the stroma isalmost entirely crushed and a narrow strip only remains between theglands. Such cases have been spoken of as benign adenomata. As,however, the epithelial cells never penetrate the basal membrane of thegland we cannot admit the term adenoma, which signifies a new growth.
