Medical professionals must keep clear and concise notes on their patients. Information gathered during an exam or interview is transferred into the patient’s record chart. Recently patient charts are digital, as data gathered is often entered into the office or hospital computer system by a nurse or physicians assistant. In America, HIPAA stands for the Health Insurance Portability and Accountability Act of 1996. The original idea was to force the healthcare industry to save money by computerizing paper records. That led to concerns over privacy and new privacy regulations from the Department of Health and Human Services (HHS).
