Oswald Hugh Davies's lorry got trapped onto a railway crossing in Whitland, Carmarthenshire which caused the accident. Up to 60 people were on the crowded service between Milford Haven, Pembrokeshire, and Manchester when it hit a farm vehicle carrying bales of hay at on the 19th of December 2011. The driver was unable to stop the train, which was travelling at 50mph, in time after spotting the lorry stuck on the crossing at Whitland, Carmarthenshire, police said. Passengers told how commuters screamed in terror as the train suddenly slammed on its emergency brakes, before they felt a sharp impact.
