A few scattered areas of evident cortical ab- 164 PYELOGRAPHY scesses connected with the pelvis are dimly visible andsuggest the wide-spread cortical destruction. 6. Alternating Contraction and Dilatation.—With a chronicinflammatory process largely confined to the renal pelvis, its outline may become irregularly contracted as well asdilated. This may be due either to contraction as the re-sult of inflammatory changes in the peripelvic tissues or. In Fig. 134 the outline of therenal pelvis and upper ureter is irregularly narrowed anddilated. The patient complained of chronic hematuria, which at operation was found to be due to a chronic pye-litis granulosa. The changes in the pelvis itself may bedue to the encroaching proliferation of the mucosa, whilethat in the ureter is probably due to changes in the peri- INFLAMMATORY DILATATION 165 ureteric and peripelvic tissue, which irregularly retractedthe ureter at the ureteropelvic juncture and its adjacentportion. 7. Atrophic Contraction of Pelvis.—An atrophic conditionof one or both kidneys is occasionally observed. Micro-scopic examination of the renal tissue often demonstratesthe existence of an etiologic inflammatory process. The
