It would be next to impossible to diagnosticate a myoma of thebroad ligament without an exploratory laparotomy. If such a tumoris revealed by an exploratory laparotomy and its removal is deemednecessary, this should be done by enucleation. Large vessels shouldbe avoided so far as possible in cutting down upon the tumor. Theenucleation is to be done exclusively with the fingers and with bluntinstruments. If the cavity is not large, the wound can be sutured afterthe hemorrhage has been arrested completely. If parenchymatousoozing is troublesome or if the cavity is large, drainage into the vaginaby means of a cross-tube, as advised by Martin and Kaltenbach, shouldbe resorted to (Fig. 366). The wound is sutured throughout with special. Infec-tion from the vagina is prevented by iodoform-gauze packing, whichshould also embrace the distal end of the tube. The drain may beremoved as soon as all discharge from it has ceased.
