Operative gynecology : . he obstruc-tion. It is greatest when the stoppageis at the extreme lower end of the ure-ter, as in the case of a calculus plug-ging: its orifice. One of the commonestforms, generally of lesser degree, is thatproduced by the pressure of large uter-ine or ovarian tumors, which is almostalways greatest just at the pelvic brim,so that the hydroureter affects the abdom-inal portion only. A marked case was found in one ofmy patients (S. A. H., 4039, Dec. 30,1895), who died with an enormous car-cinoma of the cecum filling the wholeabdomen and pelvis. Following the dis-tention, the ureter had become kinked, and adhesions had formed which boundthe kinks together, so that the distentionwould have remained even if the causehad been removed. In one instance figured in the text (Fig. 291) the ureter was obstructed by the pressure of a sarcomatous growth of the peritoneum; as the ureter distended it kinked, and became further obstructed by a band of adhesion uniting it to the pelvis of the Fig. 291. — Hydroureter and Hydrone-phrosis, showing Kink in Ureter dueto Band of Adhesions stretching fromthe Lower Part of the Pelvis of theKidney almost down to the PelvicBrim. § natural size. Sarcoma of 4, 1895. H. B. Autopsy 536 AFFECTIONS OF THE URETERS.
