I have entered the army July 26th 1861 as Asst: Surgeon 1st Regt. Ills. Cav. Vols, and was mustered into service in August 1861 we marched from St Louis Mo. for Jefferson City Mo, where I had the misfortune of being thrown by a horse, which resulted into a luxation of my left shoulder. from here we were ordered to Lexington Mo, at which place we had engagement with the rebel army under Price September 12th 1861, and here I was struck by a spent ball in the abdomen, severeally bruising the parts and resulting in suppuration, causing adhesions of the covering of the internal organs. Although suffering considerable at the time, my services were very much needed and I remained on duty during the siege, and after the surrender I was sent to St Louis Mo, where I performed duty at one of the General Hospitals, until February when Dr. B Wright Surgeon ?s[?] Medical Director on Maj. Gen. Hallecks[Halleck ?s] Staff, recomanded me for a promotion. His Excellency Gov. Gamble of Missouri, appointed me Surgeon 23rd Regt. Mo. Infy Vols. With this Regiment I was in the battle of Shiloh Tenn. where we lost two thirds of the Regiment, and with the remaining part, I was ordered to Alton Ills, and ever since, I have been stationed with the Regiment, at various at various Posts in the Dept. of the Missouri, until October 27th 1863 we marched from Rolla Mo to the Dept of the Cumberland, and here in active duty on fatigues marches. I suffered very much, from a severe pain in the region where I was wounded, especially while riding on horseback, my left arm paining also, so that but with great difficulty I could
