Amputation through the Condyles of the Femur.—In this amputation, which is known as Cardens ampu- 524 AMPUTATIONS. tation, an anterior flap, whose lower extremity is threefinger-breadths below the patella, is cut and the disarticu-lation effected; the posterior soft parts are divided, thepatella removed, and the condyles next sawed through justabove the edge of the articular cartilage (Fig. 426, B).Lateral Flap Method.—In this operation an incisionis made just below the patella, and is carried down thespine of the tibia for three inches, and is then carriedbackward to the middle of the leg at a point opposite thebeginning of the incision ; a similar flap is cut on theopposite side of the leg, and the flaps dissected up tothe line of the articulation. When this point is reached,the joint is opened and the disarticulation effected. Thepatella is not removed (Fig. 427). Fig. Amputation at the knee-joint by lateral flaps. (Smith.) Grittis Amputation at the Knee-joint.—In this opera-tion a long rectangular anterior flap is first cut and dis-sected up, and after the disarticulation has been effectedthe skin covering the posterior surface of the knee is cutfrom within outward. The condyles of the femur arenext removed by a saw above the edge of the articularcartilage, and the articular surface of the patella is removedby the saw or cutting forceps. The patella is next broughtdown, so that its sawed surface is in contact with thesawed surface of the condyles, and the flaps are broughttogether (Fig. 428, A). AMPUTATIONS OF THE 428. 525
