The malignant tumors in the kidney, as elsewhere, are those prone to localrecurrence and to generalization—the carcinomata and the sarcomata. By some writers the carcinomata are included among the adenomata, andvice versa; but, although the transition from adenoma to carcinoma is oftenhard to define to the microscopist, the clinical behavior of the two classes isvery distinct. All the varieties of carcinoma occur in the kidney as primary or secondarydeposits, but most often the primary growth assumes the medullary type and. It arises generally in the cortex of the gland, either as asingle encapsulated growth or may be infiltrating from the first; or a numberof tumors may develop in different parts of the kidney at the same time. In size the tumors vary inversely with their numbers, so that as many astwenty may be found in a single kidney, each of small size, or in other casesa single tumor may be present and reach the dimensions of a mans head orlarger. In either case the affected kidney is enlarged, and may be nodular orsmooth in outline. Occasionally parts of the tumor are found to have undergone retrogradechanges and to be replaced by fluid material, generally gelatinous.
