A system of surgery . Fig. 273.—M. Bousseau s case of Separation oftlie Upper Epiphysis of the Femur ; from aboy of 15, who was run over by a , Head ; n, neck ; p, periosteum ; c, capsule. FRACTURES OF THE FEMUR. 837 that the angle diminishes in advanced age on account of seniledegeneration (page 726). At the base the vertical diameter is twicethe antero-posterior; passing inwards the former decreases rapidly, the latter increases more slowly, and the two diameters become equala little below the head—this is the narrow part of the neck. Ina coronal section of the head and neck we note that the under sur-face of the neck is formed by a layer of compact bone of considerablethickness continuous below with the inner wall of the shaft, endingabove in numerous cancelli passing to the head. The concavity power. downwards and inwards of this arch increases, and itsof resisting vertical pressure di-minishes the lower the angle ofthe neck. From its upper convexaspect most of the pressure la-mella3 rise which radiate to thegreat trochanter, to the upper sur-face of the neck, and especiallyto the head, and which are crossedand bound together by the ten-sion lamellae from the outer wallof the shaft and upper boundaryof the section. More than half thewidth of the head is seen to pro-ject inwards bejond the extremityof this strut. An oblique sectionthrough the same parts at rightangles to the coronal plane showsthat anteriorly the compact layer thickens to the line of attachment Fig. 274.—A Femur showing moderate Ab£ ,i i / i £ sorption of the Neck, after intra-capsu or the capsule (capsular portion orspiral line). Posteriorly the compactlayer is everywhere thin, and thatcovering the roll-like intertrochan-teric line seems to overlap ratherthan to be continuous with the layeron the back of t
