After reciting all of the details of the occurrence, thereport states that the accident was caused by failure ofthe engineman on one of the trains to obey the stopindications of the automatic signal and of a flagman onthe train ahead. A contributing cause was the failureof an instructor engineman who was in the electric loco-motive cab to observe the signals and cause compliancewith their indications.
