Cafe au lait spot on the skin of an 18 year old man due to neurofibromatosis (von Recklinghausen's disease). This is an inherited disorder characterised by these spots of pigmentation and the growth of soft swelling from the nerves (neurofibromas). The neurofibromas may cause epilepsy if they grow in the brain, or rarely they may turn cancerous. The neurofibromas can be surgically removed if they are problematic.
