CT scan of a 16-year-old male who was involved in a car accident and sustained severe head injuries


CT scan of a 16-year-old male who was involved in a car accident and was not wearing a seatbelt. The CT scan showed severe head injuries. There is a scalp laceration and contusion in the right temporal region (highlighted in blue). There is diffuse hyperdensity in all the cisterns and sulci in the supratentorial and infratentorial brain with extension into the ventricles, these are consistent consistent with severe subarachnoid hemorrhage. There are multiple hyperdensity in the basal ganglia bilaterally consistent with hemorrhage (highlighted in yellow and red) . There are other focal hyperdensity in the frontal lobes in the gray-white matter interface most likely shear diffuse axonal injuries. There is moderate effacement of the lateral ventricle, third and fourth ventricle and also the basilar cisterns are effaced. There is blood in the interpeduncular foci, suprasellar cistern and CP angle cistern. There is no midline shift. There is evidence of mass-effect evident by the effacement of the sulci and cisterns. There is soft tissue contusion and swelling in the right temporal region. There multiple fractures involving the bones of the cranium and the facial region. These include the right frontal bone, extending into the temporal bone and extending anteriorly to cross the right and left greater and lesser wings of sphenoid. There is fracture of the right and left zygomatic bones. This also fracture of the lateral and medial wall of the orbital cavity on the right. There are fractures of the nasal bones. There are hematoma air-fluid level in both sphenoid and the maxillary sinuses, ethmoid air cells and frontal sinuses. At the base of skull around the atlas there is contusion and hematoma.
